I, the undersigned parent/guardian, understand that participation in the camp involves physical activity, including but not limited to running, jumping, contact with other players, and use of sports equipment. I acknowledge that these activities carry inherent risks of injury, including but not limited to sprains, fractures, or more serious injuries.

 I acknowledge these risks and voluntarily allow my child to participate.

I hereby release, waive, and discharge the camp, its directors, coaches, staff, volunteers, and affiliated organizations from any and all liability, claims, or demands arising out of or related to any injury, illness, or accident that may occur during participation in the camp.

Medical Authorization:

In the event of an injury or medical emergency, I authorize camp staff to seek medical treatment for my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.

Code of Conduct:
